The lips occupy a prominent place in the lower face, contributing significantly to the aesthetic value of the face. Lips serve not only as the gatekeeper of the oral cavity, but also function in speech articulation, kissing, whistling, and a range of human facial expressions. Lips are soft, pliable, and protruding tissues that also act as a sensory organ. Lip modification should minimize any negative impact on these features.
Idealized lip shape and size can vary based on individual, racial, cultural, and temporal societal trends. Lip morphology variations may produce a consultation for lip augmentation or reduction chelioplasty, both seemingly to achieve a more idealized lip. Currently, shapely and more voluminous lips are deemed desirable and are a source of most lip enhancement consultations. With aging the lips loose volume and shape, often becoming a part of the aesthetic facial rejuvenation process.
Anatomically, the lips and perioral complex can be defined as the area bounded by the subnasale superiorly, mentolabial sulcus (sulcus inferioris) inferiorly, and the nasololabial folds laterally. Lip shape is mostly defined by the vermillion border, red lip volume, and amount of dental lip support. Upper lip landmarks include the philtral columns, Cupid’s bow, the tubercle, and the white skin roll leading to red lip (vermillion) border. Cupid’s bow is formed by M-shaped central upper lip elevation that frames the superior border of what is commonly referred to as the pout. The lower lip is more continuous and uniform in shape. A subtle mirroring of upper lip shape can be noticed in the paramedian aspects of the lower lips. The histological layers of the lips consist of mucosa, submucosa, layers of circular muscle fibers, adipose tissue, and skin. Like other facial structures, idealized proportions of the lip have been described and may serve as a useful guide for some patients. Described aesthetic norms are mostly based on data from Caucasian aesthetic norms and may vary based on the patient’s individual features, race, and individual esthetic goals. Lip projection can be measured on lateral view photographs, where the upper lip should lead the lower lip by 2 to 3-mm. On frontal view, the upper lip should be 60-70% of the size of the lower lip. The aesthetic goal in lip enhancement depends on the patient’s request and facial features, tempered by the surgeon’s aesthetic judgment. Lip augmentation should strive to look natural and not interfere with function. Augmentation of the lip should be uniform with a bias at accentuating the central aspect of the lip. Over-augmentation of the lateral lip can produce an artificial look that is undesirable. The lip enhancement technique employed must be considered since it can influence the type of change achieved.
Lip enhancement can be achieved using temporary injectable fillers, temporary biologic implants, autogenous tissue transfers, surgical tissue advancement techniques, and with the placement of alloplastic implants.
